FOR DIGITIZED / PIFRA DISTRICTS
1.
Hand Written / Printed Application (Specimen attached) for refund request.
(Address to Managing Director, PGSHF).
2. “ATTESTED” copies of salary
slips of each claimed month clearly showing
deduction of PGSHF subscription. Last pay slip showing the stoppage of
deduction or zero deduction against PGSHF subscription is also necessary
to be attached herewith.
3. Attested copy of N.I.C

2. Applications are acceptable only by postal mail or by hand.
3. No request will be entertained for refund, if sent electronically by
e-mail, Fax or through PGHSF web site.
