REFUND PROCESS

 


PROCEDURE

FOR DIGITIZED / PIFRA DISTRICTS

1. Hand Written / Printed Application (Specimen attached) for refund request. (Address to Managing Director, PGSHF).

2. “ATTESTED” copies of salary slips of each claimed month clearly showing deduction of PGSHF subscription. Last pay slip showing the stoppage of deduction or zero deduction against PGSHF subscription is also necessary to be attached herewith.

3. Attested copy of N.I.C

FOR MANUAL DISTRICTS

1. Application Hand Written / Printed (Specimen attached) for refund request. (Address to Managing Director, PGSHF).

2. “ATTESTED” copies of pay bills of each claimed month clearly showing deduction of PGSHF Subscription. Last pay bill showing the stoppage of deduction or zero deduction against PGSHF Subscription is also necessary to be attached herewith.

3. A certificate from District Accounts Officer (DAO-1) for the claimed period / months. Specimen is attached herewith.. Specimens are attached herewith.

INSTRUCTIONS
1. Complete procedure should be followed.
2. Applications are acceptable only by postal mail or by hand.
3. No request will be entertained for refund, if sent electronically by e-mail, Fax or through PGHSF web site.
